It is not possible to extend a visitor Kazakh visa under any normal conditions. Since it is valid for only a maximum of 90 days, there are no medical requirements for the visitor visa. However, longer stay visas may require you to get a medical test report. Although, it is suggested that you get a medical check-up done before travelling to a new country.
No, it is strictly not possible or allowed to convert the visit permit into a work permit. As per the regulations, you must not seek work or engage in work during your stay. If you do require a work visa, you must return to your home country and apply for one.
