Once you have submitted the application and paid the fees, you will now have to wait to get your visa. As per your choice, you can either pick the documents and visa from the mission or VAC, or you may opt for it to be delivered to you via mail.

It is not possible to extend your Italian permit. Similar to other Schengen permits, short term visas are non-extendable. Meaning, you will have to leave the country upon expiry. If you do require another permit, you will have to apply it from your home country. However, you may be able to apply for an extension in certain extreme unforeseeable circumstances.
You may not stay for any amount of period once your permit expires. You are expected to leave the country before the visa expiry date. In case you do not leave, it is considered as a criminal offence, and you will get deported and banned for another entry. There are no mandatory medical requirements for the application of the tourist permit.
