You can extend your Polish permit by contacting the respective authority in Poland. However, note that you cannot extend the short term C category permits under normal conditions. You will be allowed to extend them only in extreme cases. Nevertheless, you may extend the long term D categories depending on the rules.
There are no mandatory medical test requirements for the short term visas. However, the long term categories may have medical tests required.
